DSN Can't create output

Lads,

This one has me perplexed..  I've been seeing this in this mornings logs:

messages
May 27 04:58:27 ny-cobalt-2 sendmail[803]: i4R8wMc00781: i4R8wRl00803: DSN:
Can't create output

Have I got some sort of a misconfig problem or has something broken?  I've
plenty of disc space and other mails are being delivered.
