No, it's a batch based processing system, calling it on an individual
message - or while a user is waiting for a download is likely to cause
unacceptable load and delay

Programming in C won't help much, MailScanner is written in perl.

If... Your users always connect to their pop servers through your
gateway you could think about creating mailboxes for them on the server
(which they can access through pop) and pulling their mail down through
mailscanner using something like fetchmail.
