I think your confusing milter-sender with milter-ahead.  Milter-sender
just does call backs to validate the sender not the recipient.

Milter-sender works via SMTP. It does a call ahead to verify the RCPT
with the next hop. It relies on the mailterable. So, to answer your
question, it ca work on vintage Exchange so long as your servers
supports SMTP.
