if you use a recent version of sendmail, then everything will be scanned.
By recent I probably mean 8.11 or 8.12. Earlier versions deliver mail that
was created by invoking /usr/sbin/sendmail without it ever actually hitting
a queue. The recent versions behave in a better way, so the message always
gets put into the incoming queue.

The other alternative is to stop your mail client software from ever
invoking sendmail directly. Most email apps (or web mail apps, etc) can be
configured to send mail via a remote SMTP server. Do that, and set the
server name to "localhost" and you'll be away.
