Okay, I was able to figure out why the CPAN issues were happening, and resolved it.
If ICMP is disabled on our edge firewall (a Cisco ASA5540) for this particular machine, the MailScanner update process would fail.
I ran the upgrade process again, and it worked without exhibiting any errors.
However, MailScanner still does NOT seem to be working as it should, because each email that comes in, is simply handled by Sendmail only.
MailScanner does not do anything, i.e., there is info in the maillog to indicate that it scanned the incoming email at all.
It starts okay, and logs during startup, but after that, nothing.
I have no idea what is going on. Where should I start, in debugging what is going on?

However, the install seemed to complete okay, and I restarted all of the necessary services, but I still do not ever see MailScanner processing any new emails as they come in. When I restarted MailScanner, and viewed the maillog, and all that I see is the standard startup banners, but after that, MailScanner never does anything.

The distribution specific packages are no longer tarballs and are distributed entirely as a package.
To update your system:
rpm -Uvh MailScanner-5.3.3-1.rhel.noarch.rpm
/usr/sbin/ms-configure --update
