I ran into this same problem. My fix was that quarnatine was not 
writable by the user MailScanner was ran as. Make sure this file was 
actually written to.
" Saved entire message to 
/var/spool/MailScanner/quarantine/20120501/q41FtFQd003086 " In my case 
everything looked good but in the end it was bad permissions on the 
quarantine dir.
