Mailscanner v3 had NOT been supported for many years....
I'd start with installing mailscanner v4.latest as fresh install and go from there..
You don't mention how much email (number of and volume), but 1GB per CPU core is recommended due to SA being a real memory hog!
Clamd works nice for many people and is supported in the latest MS version.
As for spam - more than likely SA 3.2.3 WITH the SARE rules etc will really make a difference..
