First of all, you should always stick to the ports if possible. It will 
ensure things just work, and it will be easier for you to manage and 
keep the software up to date. As for the ClamAV perl module, last time i 
tested it, it worked just fine without a threaded perl. All in all you 
should probably test it yourself before deployment, you can recompile 
perl with thread support. For any further questions, i would go to the
