And if you use Razor, have you done a "razor-admin -discover" recently? 
That has bitten people before.

Set the two debug options in MailScanner.conf and see where it is 
getting held up. When it sticks, thump Ctrl-S to pause the output and 
see what it was doing. Anything to do with cloudmark.com is Razor. 
Ctrl-Q will make it carry on.
