After more poking around in the wiki I found a page about releasing
emails from quarantine at
http://wiki.mailscanner.info/doku.php?id=documentation:configuration:mta:postfix:how_to:release_quarantined_mail

I found that the key ingredient that I was missing appears to have been
that the file permission must be changed:

Releasing an email from quarantine is a simple matter of replacing the
queue file back into the Postfix queue, so that it gets sent. The
permissions on the file are wrong, however and must be changed.

# chmod 700 <filename>

Doing that fixed it. I never would have guessed that! Sorry for spamming
the list.
