any idea why this mail might still have got marked as spam?

Um, no. The "envelope sender" is the sender address as given in the
SMTP conversation (MAIL FROM:<envelope at sen.der>"). It can, and often
do, differ from what is given in the mail headers. Take a look at
http://wiki.mailscanner.info/doku.php?id=documentation:test_troubleshoot:mta:connexion
for some examples of what the SMTP conversation might look like...
Conversely, the envelope recipient(s) are given with the "RCPT
TO:<envelope at recipie.nt>" line(s).
