Just portupgrade. It will upgrade your MailScanner.conf and produce 

/usr/local/etc/MailScanner/MailScanner.conf.new.versionnumber

You can either simply rename this to MailScanner.conf or diff it to see the changes etc.

All languages.conf files should upgrade automatically as well. The wrappers and autoupdate scripts will not be updated automatically (working on this new version of the port though). You can either

make renew-wrapper
or
make renew-autoupdate

in /usr/port/mail/mailscanner to update those. This will not work with portupgrade though so if you want this you do

cd /usr/port/mail/mailscanner
make
make deinstall
make install
make renew-wrapper
make renew-autoupdate


Hope this helps. BTW: The current FreeBSD stable port is at 4.38.10.
