In simple terms:

RBLs in MailScanner.conf - if mentioned these are called directly by MS and
email is scored

RBLs in spam.assassin.prefs.conf - overrides default SA scores

RBLS in SA itself - builtin with the installed version of SA. Default rules
/ scores
