Wow... I wrote him after I saw that message, and he replied that the
form was set up 5 minutes ago.  The form is effectively available now.

So, the address for looking if an URL is listed is:

http://www.rulesemporium.com/cgi-bin/uribl.cgi

And the address for submitting is :
http://www.rulesemporium.com/cgi-bin/uribl.cgi?report=1
