MailScanner as proper Postfix content_filter?

Simply, no! More specifically, MasilScanner uses 'batch' processing to
minimise scanning overhead. There is no easy way this can be done by using
any SMTP/ LMTP interface (Like AMAVIS). All that would be created is
additional over head of the SMTP transport recieving the message and
putting it in a queue directory for batch scanning than being returned to
an 'out going' queue directory to be given back to Postfix.

AMAVIS differs in as much as it looks to process messages using daemon
scanners on an 'as recieved' basis. I have used both (Side by side at one
point) and prefer MS because it has more features such as web bug
disarming, html content inspection etc. As far as pure virus scanning and
spam scanning are concerned there is little to choose between them. One
advantage that AMAVIS does have is that you can use it to 'pre-scan' mail
at SMTP stage using the new smtpd_proxy_filter option in 2.1.x. (But not
for busy servers).

The queue files are less messed with using the hold method as detailed at
http://www.sng.ecs.soton.ac.uk/mailscanner/install/postfix.shtml as MS
doesn't interfere with the Postfix queue runner process.
