spamd is configured to have only 5 children at a time and you get too much 
mail for this

I think this indicates the milter is now completely dead, sendmail is 
bypassing the milter (if correctly configured).

I think you are doing it the wrong way. You either need to use:

- milter + spamd
- MailScanner + SA

NOT BOTH !
