inconsistent SPF - bug

In the interest of completeness, thanks also Chris Picciotto for lots of
testing and ideas.  Also, note that the workaround I suggested in an
earlier message of using sendmail to add an "X-Envelope-From" header
(also suggested in the SpamAssassin USAGE document) doesn't work 100% of
the time.  The problem is that sendmail won't add the header if it
already exists in the incoming message.  Chris developed a working
method of using sendmail by adding a uniquely named header instead, and
then specifying it with "envelope_sender_header".  The patched version
of MailScanner appears to overcome these problems.
