Did you run SAR?
Are your disks thrashing?

Running top adds a bit of load to your system, but gives you a VERY good
idea of what's getting your CPU cycles.

How many instances of mailscanner do you see running?

On my (ram limited) system, I had to reduce the number of concurrent
mailscanner processes in order to get good performance. This may not be
your problem, but when that change was made here, the queues got handled
much more quickly.

(The "Max Children" setting is in /etc/MailScanner/MailScanner.conf)
