Okay, the important thing is to trace whether mail is being queued into
/var/spool/mqueue.in.

Reading between the lines on your post it looks like you have your
user's mailboxes on the same machine as MailScanner.  Can I just check
with you that the emails that aren't being tagged are in fact coming
into your mailboxes from an external source?  Mail between users on your
system quite possibly isn't going to get passed through MailScanner.

You also mentioned LMTP.  are you still using SMTP to receive incoming
mail?
