Normally that would not be done in SpamAssassin but in the "glue" that 
uses/calls SpamAssassin like MIMEDefang, AmavisNew, MailScanner, etc. 
Or you can do it at the MTA with a milter.  Since you didn't tell us any 
details about your environment, it's tough for us to guess.

included with a question like this.

I mainly use MailScanner which has settings to do exactly what you are 
asking to do.  I understand that MIMEDefang is very powerful and 
flexible and AmavisNew has a banned_filename_re setting that I just 
found with a quick Google search.
