You could use a rule like (off the top of my head, so don't shoot me if
it isn't correct):
ifplugin Mail::SpamAssassin::Plugin::MIMEHeader
mimeheader LOCAL_ATTACHED_PDF Content-Disposition =~
/filename=\".{1,32}\.pdf\"/i
score LOCAL_ATTACHED_PDF 0.01
endif
