We have a few University clients who are using Milter-ahead, a subset of
Milter-sender, with no problems.

We are planning to incorporate Milter-sender and several of Anthony Howe's
other milters in the next major release of DefenderMX. All of his milters
are very solid software. 

The best part of using Snertsoft software is Anthony's support :)
