Some mail (up to 7 days old) is stuck in /var/spool/mqueue

Have you checked /var/spool/mqueue to make sure there is a corresponding 
"d" and "q" file for the email?

for example: take the first email in your mailq results, you should have 
a file named "d6JIfxwS020007X" and a corresponding "q6JIfxwS020007X"
located in /var/spool/mqueue
