Missing file with 4.45.4-1

I have just installed on a new RHEL4 box and it all installed just fine. 
I don't know where your i386i386 is coming from. You should find you 
have a 
/usr/src/redhat/RPMS/i386/MailScanner-perl-MIME-Base64-3.05-5.i386.rpm 
which you can just install using "rpm -Uvh" manually.
