MailScanner occasionally not picking up from the hold queue

I would first off collect some mail and then run MailScanner in debug
mode (See MailScanner.conf towards the end). This will output what
MailScanner is doing to screen. Check for anything obvious such as
delays or errors. That should give you a starter for 10.
