Do I run this in foreground or can I continue to run this in the background (a entry in mailscanner.conf).

You may want to debug this:

X-mmtnet-MailScanner-SpamCheck: not spam,  SpamAssassin
> (Disabled due to 20 consecutive timeouts)

If SpamAssassin isn't scanning it, then it will be hard to get it marked as Spam
