Possible bug with spam permissions

Yep. I misread WorkArea.pm.

Correct. We agree and this is exactly what is not happening.

Run As Group = mail

Even if I am not able to provide the correct code segment, the current code clearly is wrong. Gid should correspond with "Quarantine Group" but it corresponds with "Run As Group".
