All mail treated as spam?

Because that's the definition of what the mailscanner spam lists do. If
listed, tag as spam, no matter what SpamAssassin has to say about it.
Period. No scoring, No nothing. 1 hit = instant spam tag.
Basically the spam-lists are intended for those who want to just use
RBLs as a block criteria. If you want to use scoring, then do it in your
SpamAssassin configuration.
