Permissions on /tmp are right:
stat /tmp

And perl -w /usr/lib/MailScanner/clamav-autoupdate shows no errors.

Does "perl -w /usr/lib/MailScanner/clamav-autoupdate" show any errors?
Also what are your /tmp directory permissions? (Should be 1777) you can see
this by issuing "stat /tmp"
