mailscanner with postfix startup messages

Setup is postfix/mailscanner/sa used as a gateway. I seem to have an issue
with some messages getting a "hold: header Received:", being requeued then
basically "sitting" there, not being delivered.
Can any one explain to me what the "hold: header Received:" indicates and
what causes it?
Also, the "qmail 34926 invoked by uid 60001" entry has me perplexed. Where
did that come from?
