Mailscanner-4.28.6-1 and SPAM Actions= attachment deliver problems

Exactly what resulting message are you expecting?

What "attachment deliver" (it doesn't matter about the order) will do is
take the original spam message, and turn it into an RFC-822 attachment of a
new replacement message. The body of the new replacement message will say a
bit about what happened, give the From+To+Subject information, and include
an attachment which is the original message. Not all mail apps can actually
handle RFC-822 but Outlook handles them perfectly. In Outlook you can open
the attachment and it appears as a complete new message, which is exactly
what should happen.

What do you think should be happening?

I will take a look at adding code to insert the SA report at the beginning
of the message body. No guarantees though.
