Debian Testing/Unstable, Exim4 and MailScanner 4 [SCANNED]

I use the debian package and have looked at README.exim4:
 
"I have also had a look at README.exim4, but it doesnt seem to say anything about getting the daemons started with init.d, which is where i went wrong I think."
 
...and when I followed the instructions, I have again got mailscanner working but mail just seems to sit in the outgoing exim queue directory which makes me think the second exim daemon is not starting properly. 
 
I'm not quite a complete newbie, but neither am I anywhere near being a guru, so if somebody could spell out this last step to me I would be very grateful. The information in the package up to this point has been first rate, but unless i'm missing something (very probable), I can't get the second daemonto deleiver the mail.
