Whitelist on word documents

Ok, I faq'ed, maq'ed, and googled until I'm green.   Still can't figure
out a way to whitelist based on a file attachment extension.   I want to
whitelist messages with a .doc attachment,  ie ms word.

Running Mailscanner with Sendmail and spamassassin.
