A virus inside a zip file inside another zip file of SAME name is not discovered

What version MailScanner are you using? I assume its a little older
version?

There were some changes recently to avoid this behaviour, and also most of
the virus scanning engines were updates to support this. Did you update
both recently ? (Your AV solution and MS ?)
