WHy was this not caught??

Just wondering why this was not caught by MS, as it even says in the header
"(may be forged)"

I seem to be getting more and more spam now that is not caught even after
adjusting my settings to 4 rather than 5 for a hit and deleting spam over a
score of 10

Anything I am doing wrong or not doing?

Any help appreciated...
