4.25-6, whitelist not working?

   I had several outbound messages tagged as spam yesterday.
My whitelist rules have looked like:

From:   137.146.        yes
FromTo: default         no
To:             spam@*  yes
To:             abuse at colby.edu yes
To:             postmaster at colby.edu    yes
To:             career-services yes
FromTo: owner-victoria at LISTSERV.INDIANA.EDU     yes
FromTo: lists.worldbank.org     yes
FromTo: alum.colby.edu  yes
FromTo: nytimes.com     yes
FromTo: spamcop.net     yes

for a long time, where my domain is 137.146.x.x.  Any suggestions?
Should the default fromto be at the bottom of the list?
