deny all file extensions

You want to put the allow rules ahead of the deny rule. Then you could just
ban any file whose name includes a "."
deny    \.      Files banned by default Sorry, most attachments are banned

Make sure each chunk of that line is separated from the next by tab
characters and not spaces.
