I think of it more as 'lines of defence'

1st line - E-mail Gateway (running MailScanner/SpamAssassin/F-Prot)
2nd line - Client's PCs

Having a AV solution with MailScanner hasn't hampered e-mail flow. If I
were a client I would prefer my ISP to handle dodgy e-mail, especially if
I'm paying hosting.
