I've had it happen twice...

Once it was caused by a virus update not completing (clamav actually),
which resulted in the lock not being released which prevented scanning
from taking place.  As there isn't a wrapper to update command I doubt
this is your problem (unless command has its own updater which locks its
scan engine?)

The other time it was caused by SpamAssassin's auto-whitelist capability
(which should have been turned off anyway)
You might try turning off Spam Assassin if you are using it to see it
that helps, if so try disabling auto-whitelisting and bayes
classification before re-enabling it.
