MailScanner uses the lists such as ORDB to tag spam. It doesn't prevent
delivery, it just tags the messages with an extra header (and optionally
some extra text at the start of the Subject: line).

If you want to completely prevent delivery of messages from hosts listed in
ORDB, then you'll have to configure sendmail to do it.
