Mailscanner locking up

I have put it into production, and according to the laws of Murphy,
something dies.

mailscanner starts, reporting into mail.log syslog that it is running --
that is the last message I see.

Our SMTP workload is very very heavy, with 19000 accounts.  Even doing an ls
-l on the /var/spool/mqueue.in directory takes forever... sendmail is still
running, in the recommended configuration.

If anyone has seen Mailscanner challenges like this in the past, I would
appriciate any help.
